Pharmacy/CHW Collaboration (HSM)

Description Registration

Patients visit pharmacies more than any other health care setting. The work of Community Health Workers (CHW) and advanced pharmacy technicians in community pharmacies go hand-in-hand. This course discusses 1) the benefits of CHWs collaborating with pharmacies to provide services that improve the health of all patients, and 2) the need for CHWs in pharmacy practice, and the services that can benefit patients and their healthcare outcomes.

Course content covers:

- Healthcare services offered in community pharmacies, and the role of advanced pharmacy technicians in provision of services - Patient benefits with pharmacy technicians and CHWs, and collaboration opportunities - Assessment of community needs and health education strategies to address social determinants of health and address health disparities - Reporting and documentation during care coordination

Continuing education credit (2.5 hours) is available for pharmacists and pharmacy technicians completing this module.

Students are encouraged to complete this module within two weeks. HSM access is for six months. A Certificate of Completion can be printed after successful completion of the course.
