Description Registration

The purpose of this course is to provide an understanding of Heart Failure. As a Community Health Worker, it is important to understand some of the risk factors your patients may have and address the barriers that your patients face. Heart failure is a chronic condition that directly impacts between 5 and 6 million Americans, as well as their caregivers.  It is generally a chronic condition that does not go away.  Students will apply knowledge of Heart Failure in order to successfully navigate patients through the healthcare system.

Students are encouraged to complete this module within two weeks. HSM access is for six months. A Certificate of Completion can be printed after successful completion of the course.
