Pharmacy/CHW Integration into Practice (HSM)
Community Health Workers (CHWs) can be game changers for a community pharmacy practice and the patients they serve. This course provides the business case, the patient case, and a step-by-step approach to integrating a community health worker into day-to-day pharmacy practice.
Course content covers development of a job description for a pharmacy-based CHW, revenue sources, areas for enhanced pharmacy services with CHWs, and establishment of a sustainable model for CHWs within pharmacies.
Continuing education credit (1.5 hours) is available for pharmacists and pharmacy technicians completing this module.
Students are encouraged to complete this module within two weeks. HSM access is for six months. A Certificate of Completion can be printed after successful completion of the course.
